Behavioral health care includes diagnosis and treatment for mental health and substance use conditions. Medication and therapy are common treatments for these conditions, but they’re not the only types of care. Each client’s specific treatment plan depends on their unique needs and symptoms, based on a diagnosis from a trained professional.

Momentum for Health is one of the largest nonprofit providers of mental health services in Santa Clara County. Our team specializes in the unique needs of people with behavioral health conditions. We have the training, skills, and experience to support clients on their journey to recovery.

Our mission is to help individuals achieve mental and emotional health, discover and reach their potential, and fully participate in life. We carry out this mission through a range of outpatient, residential, and supportive services as well as community programs.

Our adult programs serve clients over age 26.

Momentum’s services are client-driven, so treatment plans are customized to each person’s specific needs and the goals they set. Every client receives whole-person care to support their physical, emotional, environmental, and social well-being. Clients remain in charge of their recovery with our staff as their supportive partner.

Momentum for Health accepts Medi-Cal and uninsured clients.

The La Selva Group, a division of Momentum for Health, accepts clients with commercial insurance, also called “private pay.”

Our team partners with you to develop your customized treatment plan. Your treatment varies depending on your specific needs, symptoms, and goals. Your dedicated case manager helps you navigate every step of recovery.

Your case manager will help you find appointment times and locations that work for you. Your options will vary based on your specific situation and might include:

• Online treatment through telehealth
• Appointments at your home or other locations in the community
• Treatment at Momentum for Health locations in Santa Clara County
• Momentum’s rideshare program, which offers transportation to treatment, medical appointments, food banks, and group activities

Our staff includes psychiatrists and other medical services staff who can provide medication management as needed.

Momentum offers independent and supportive housing services for eligible clients who are receiving care within our programs, depending on availability. We also provide adult residential services designed to empower clients to re-enter the community and live independently.

Having a family member or friend in behavioral health treatment can feel confusing and overwhelming, but you’re not alone. Many of our team members are past clients of Momentum for Health or have loved ones with behavioral health conditions.
