What is behavioral health?
Behavioral health includes mental health and substance use disorders that affect a person’s mood, thoughts, actions, and relationships. These conditions often impact every part of someone’s well-being: mind, body, and spirit.
For people with mental health and substance use disorders, getting the right care as soon as possible is very important. That’s our focus at Momentum for Health.
What is behavioral health care?
Behavioral health care includes diagnosis and treatment for mental health and substance use conditions. Medication and therapy are common treatments for these conditions, but they’re not the only types of care. Each client’s specific treatment plan depends on their unique needs and symptoms, based on a diagnosis from a trained professional.
What is Momentum for Health?
Momentum for Health is one of the largest nonprofit providers of mental health and addiction treatment services in Santa Clara County. Our team specializes in the unique needs of people with behavioral health conditions. We have the training, skills, and experience to support clients on their journeys to recovery and healing.
Our mission is to help individuals achieve mental and emotional health, discover and reach their potential, and fully participate in life. We carry out this mission through a range of outpatient, residential, and community programs.
What ages does Momentum serve?
Our adult programs serve clients over age 26.
How does Momentum for Health approach treatment?
Momentum’s services are client-driven, so treatment plans are customized to each person’s specific needs and the goals they set. Every client receives whole-person care to support their physical, emotional, environmental, and social well-being. Clients remain in charge of their recovery with our staff as their supportive partners each step of the way.
How do I access care at Momentum?
Admittance varies by program and usually starts by contacting the Santa Clara County Behavioral Health Call Center at (800) 704-0900 or your health care provider.

What types of insurance does Momentum accept?
Momentum for Health accepts Medi-Cal and uninsured clients.
The La Selva Group, a division of Momentum for Health, accepts clients with commercial insurance, also called “private pay.”
How long does my treatment last?
Our team partners with you to develop your customized treatment plan. Your treatment varies depending on your specific needs, symptoms, and goals. Your dedicated case manager helps you navigate every step of recovery.
Do I need to come to the office for treatment? What if I don’t have transportation?
Your case manager will help you find appointment times and locations that work for you. Your options will vary based on your specific situation and might include:
- Online treatment through telehealth
- Appointments at your home or other locations in the community
- Treatment at Momentum for Health locations in Santa Clara County
Can Momentum help me manage my medication?
Our staff includes psychiatrists and other medical services staff who can provide medication management as needed.
Does Momentum provide housing for clients?
Momentum offers independent and supported housing services for eligible clients who are receiving care within our programs, depending on availability. We also provide adult residential services designed to empower clients to re-enter the community and live independently.
My loved one is receiving care at Momentum. What do I need to know?
Having a family member or friend in behavioral health treatment can feel confusing and overwhelming, but you’re not alone. Many of our team members have their own personal experiences with behavioral health conditions, and some are even former clients of Momentum for Health.
Talk with our team about support groups that may be available to you. It’s important to take care of yourself, physically and emotionally, while your loved one is in treatment.
