Comprehensive care customized to each client’s needs.
As one of the largest behavioral health programs in Santa Clara County, Momentum for Health’s adult outpatient mental health services provide comprehensive care customized to each client’s needs. This care takes place via appointments at one of our four locations in San Jose and Mountain View. Appointments can include everything from diagnosis and psychiatric care and medication management, to ongoing therapy, rehabilitation counseling, and referrals to other Momentum programs, including supported housing and employment services.

Our whole person care approach is designed to support your mental, emotional, physical, and social well-being. Outpatient services may include:
- Assessment and diagnosis
- Psychiatric care and medication management
- Individual or group therapy
- Skill development
- Case management
- Employment services
- Independent or supported housing
Service Details
Who is Eligible?
Our outpatient program offers behavioral health services to adults at locations in San Jose and Mountain View. Clients must meet the following criteria:
- You are at least 18 years old.
- You have a behavioral health diagnosis, such as Major Depressive Disorder, Schizophrenia, Bipolar Disorder, and/or another diagnosis not listed here.
- You live in Santa Clara County.
- You have active Medi-Cal.
- You are referred to the program by the Santa Clara County Behavioral Health Call Center.
Our Care Team
Momentum for Health works with providers with a variety of certifications in our programs, including case managers, peer support specialists, occupational therapists, nurses, therapists, and psychiatrists.
What to Expect
To receive outpatient care, clients should expect to follow these steps:
- Once you have gone through the Santa Clara County behavioral health call center, Santa Clara County will process your referral to our program.
- We will meet with you to learn about your history and assign you a care team. Your dedicated case manager will support you throughout our time in the Adult Outpatient Program.
- You will receive an individualized care plan following a thorough assessment by one of our clinical staff members.
- Depending on your needs, you may visit our program once a week (or as needed) for meetings with your case manager to support your mental health, employment goals, medication management, or housing needs.
