ABOUT
A safe place to call home and the stability to build toward independence.
Momentum for Health provides independent and supported housing for current Momentum clients who are working toward recovery from mental health or substance use conditions. These programs provide safe, stable housing while helping clients build the skills needed to live independently and thrive in the community.
SERVICES & SUPPORTS
Independent Housing
Independent housing is for those who need stable housing but do not require on-site clinical care.
- Case management
- Crisis intervention
- Group or individual rehabilitation services
- Medication support services
In Independent Housing:
- Clients are expected to live independently with access to a private room and shared common spaces.
- Clients are responsible for utilities and groceries.
- Clients will receive support tailored to their lifestyles from Housing Support staff.
- Clients will be able to live independently in safe and secure living environments with access to Momentum for Health’s wraparound services.
SERVICES & SUPPORTS
Transitional Housing
Transitional housing is designed for clients who benefit from on-site clinical care while building independent living skills. In transitional housing:
- Clients have access to a shared pantry that includes basic foods like bread, eggs, and milk.
- Clients also have access to cable, internet, and laundry facilities.
- Housing is paired with access to clinical services from other Momentum for Health programs.
- Staff are on-site Monday through Friday from 8:30 a.m. to 5:00 p.m.
- Clients will find a structured environment designed to help them build independent living skills while having access to weekly house meetings, grocery trips, gym outings, community events, and other client-led activities.

- Mental health rehabilitation
- Appointments with a primary care physician and/or psychiatrist
- Employment services
Independent Living Skills
- Housekeeping
- Money management
- Nutrition and meal preparation
- Personal hygiene
- Shopping
- Using public transportation
- Navigating community resources
Service Details
Who is Eligible?
To access housing, clients must meet the following criteria:
- You are at least 18 years old.
- You have active Medi-Cal in Santa Clara County. We cannot accept private insurance.
- You are a current Momentum for Health client actively pursuing behavioral health treatment. Housing is not available to members of the general public who are not Momentum clients.
What to Expect
Our housing programs allow clients to focus on their treatment plan, rather than where they’re sleeping at night or where their next meal is coming from. Your experience with independent and transitional housing might look like:
- First, you will have an interview to assess your eligibility. If demand exceeds available housing, you may be placed on a waitlist based on application date.
- Once admitted, you will move into your space, meet your neighbors, and meet regularly with your housing specialist.
- You will have access to coordinated care that supports your mental health, physical health as well as assistance with employment and benefits when needed.
- Clients are expected to be good neighbors by respecting others, communicating in healthy ways, maintaining appropriate boundaries, and caring for their living spaces.
- You and your housing specialist work together to determine how long you need to stay in the program.
- When you are ready, your housing specialist supports your transition to the next level of housing.
