A Supportive, Home-like Alternative to Hospitalization.
Momentum for Health’s Crisis Residential Treatment programs offer a home-like alternative to hospitalization for adults in a behavioral health crisis, usually for about 30 days. While you’re here, you work closely with our team to stabilize your symptoms, learn coping skills, and build support before transitioning back into your community.

Crisis Residential Treatment
The Crisis Residential Treatment program is designed to provide intensive behavioral health support in a home-like environment as a voluntary alternative to hospitalization. These supports include:
- Psychiatric evaluation
- Customized treatment plans
- Medication management support
- Individual and group therapy
- Life skills classes
- Benefits specialists to help you apply for public benefits
- Discharge planning to link you to additional resources after your stay with us
Service Details
Who is Eligible?
You might be eligible for Crisis Residential Treatment if:
- You are at least 18 years old.
- You have a behavioral health diagnosis.
- You can live safely in a community-based facility.
- You have active Santa Clara County Medi-Cal or no insurance.
- You are referred to the program by a case manager, hospital worker, or custody worker.
Our Care Team
The Crisis Residential Treatment team is staffed by nurses, psychiatrists, therapists, case managers, peer specialists, addiction specialists, and other dedicated behavioral health practitioners. Our programs are licensed by Community Care Licensing and certified by the Department of Health Care Services as social rehabilitation programs.
What to Expect
Here is what a client’s experience with the crisis residential treatment program might look like:
- You must be referred to the program by your case manager, hospital worker, or discharge coordinator.
- After your referral form is approved, an intake coordinator will contact you to schedule a brief interview.
- You will be placed at one of our four Crisis Residential Treatment locations in Santa Clara County.
- Your stay may last about a month. Our goal is to help stabilize your symptoms and build support to help you transition back into your community.
