ABOUT

A Field Response Team Addressing Behavioral Health Challenges in the Community

Momentum for Health has partnered with Pacific Clinics and the County of Santa Clara to pilot TRUST to expand behavioral health response that does not include law enforcement as the initial responding party. The TRUST team members meet in person with individuals experiencing low-level behavioral health symptoms to assess their medical and behavioral health needs.

TRUST is a non-law enforcement community service provided by Momentum for Health, Pacific Clinics, and Santa Clara County.

SERVICES & SUPPORTS

TRUST Field Response Program Services

We bring the support to you. TRUST sends trained professionals to respond to community members with behavioral health symptoms when law enforcement is not needed. A community-based program led by Momentum for Health and Pacific Clinics, TRUST helps Santa Clara County residents by providing a compassionate, non-law-enforcement response to behavioral health events. Response is case-by-case, but may include:

  • On-site crisis support and stabilization
  • De-escalation and emotional support
  • Safety planning
  • Referrals to mental health services and community resources
  • Follow-up support when appropriate

Get Help 24/7, 365 Days a Year

TRUST is a lifeline that operates 24 hours a day, 7 days a week, 365 days a year (including holidays). Translation into multiple languages is available.

To get help, call 408-596-7290 or 988 to access the field response team as well as on-demand mental health resources, including:

  • Immediate, over-the-phone symptom management and de-escalation support
  • Guidance to referrals and additional services
  • In-person response services that send a behavioral health professional to locations throughout Santa Clara County

We treat every situation on a case-by-case basis, providing responsive, personalized support to individuals and families during times of distress. This looks like:

Field response teams include behavioral health staff and experienced professionals trained to provide trauma-informed care. The response team will consist of at least two individuals knowledgeable about community resources, behavioral health conditions, and de-escalation techniques, and who may have lived experience to draw on. The team is diverse in knowledge, language capacity, and cultural humility.
