Momentum created the Intensive Outpatient Program to fill a need in our community—a support network for clients with behavioral health conditions who are leaving jail. 

We’re here for our clients during the first 120 days after they leave the justice system. We can help with almost everything related to your justice case. This may include:

  • Picking you up from jail 
  • Getting important items like medications, clothing, and a phone
  • Coordinating your care with pretrial, parole, and probation officers
  • Transporting you to and from court dates and court-ordered classes 
  • Building skills that you need to reenter the community, such as managing medication 
  • Helping you apply for government benefits 
  • Helping you find a place to live

What to Expect:

    You’re referred to our program by an employee in the justice system.

  • 2.

    After your referral is approved, one of our staff interviews you while you’re still incarcerated.

  • 3.

    If you’re accepted to the program, your case manager arranges your care.

  • 4.

    Be accepted as you are, without pressure to commit to any decision or action

Who Is Eligible?

To be eligible for crisis residential services, you must meet the following criteria:

  • You are at least 18 years old.
  • You have a behavioral health diagnosis.
  • You have an active justice case in Santa Clara County.
  • You have a referral from a justice services behavioral health provider.

Step by Step

All referrals for the Intensive Outpatient Program must come from a justice services behavioral healthcare provider. If you’re a justice services provider and have a client you’d like to refer for care, please contact 408-510-7080.

Our Intensive Outpatient Program team includes caring, experienced case managers. Many of them have personal experience navigating the justice system, which means they understand what our clients are going through.

Momentum’s Intensive Outpatient Program helps clients learn to thrive outside the justice system. This program is designed to help our clients stay out of the system in the future.

Day in the Life

As a client enrolled in our Intensive Outpatient Program, you have access to a case manager who helps you navigate life after being incarcerated. From picking you up upon your release, your case manager will help you find housing, clothing, and a cell phone, and will provide transportation to and from court dates and other appointments.

You don’t have to come to our office to meet with your case managers. We come to you, whether at your home, an encampment, or somewhere else you’re staying. Our team helps you get medications, therapy, and medical care, so you can rejoin the community.
