Meet with a certified addiction counselor to assess the appropriate level of care
Momentum for Health’s Addiction Treatment Services (ATS) serves adults with substance use disorders—providing supportive outpatient, intensive outpatient, and recovery services.
Our certified addiction counselors and specially trained team can help with:
You’ll work with our team to find the best options for your recovery. We offer individual counseling (both in person and virtually), group counseling, and case management.
Substance use can be complicated, and your situation may have several layers. Every level of care at ATS includes treatment for substance use along with mental health conditions (known as co-occurring conditions).
Our staff is also trained in the “biopsychosocial” approach to healing. This means we look at your whole life, from your physical and mental health to your relationships and environment, to build an action plan that’s right for you.
ATS offers outpatient services only. We can refer you to the right resources if you need more acute levels of care, such as inpatient services or withdrawal management.
ATS provides a safe space to explore and manage your substance use. The road to recovery may seem daunting, but we’re here to walk with you. As part of your treatment at Momentum, you will:
Meet with a certified addiction counselor to assess the appropriate level of care
Work with a primary counselor to customize your treatment plan
Touch base with your counselor at least twice a week, for 90 days
Be accepted as you are, without pressure to commit to any decision or action
To be eligible for Addiction Treatment Services, you must meet the following criteria:
Momentum clinicians, staff, and case workers work as a team, so you receive all the services you need. Our team coordinates with your other healthcare providers, such as your primary care provider and mental health professionals.
Your recovery might have stops and starts. If you choose to stop treatment for any reason, you’re met with support and understanding. We also make it easy to return, with no waiting period or barriers to re-entry.
We remain your partner throughout your journey, even after you finish treatment. Our staff links you to aftercare resources, such as support groups and other community services.
If you are a Momentum client, speak with your case manager for a referral.
If you’re not in the Momentum for Health system, contact Santa Clara County call center at (800) 704-0900.
We schedule an intake assessment to collect and review your basic information. Everything you share remains 100% confidential.
A credentialed counselor assesses whether you’re eligible for substance use treatment.
If you need medical detox services, we can arrange your transfer to a sobering center.
Addiction Treatment Services has a dynamic team that consists of certified/registered counselors and clinicians. Our counselors are credentialed through the California Consortium of Addiction Programs and Professionals (CCAPP) or through the Board of Behavioral Sciences.
Addiction Treatment Services is for clients who are recovering from substance use but don’t need withdrawal management or residential services. The average length of stay for outpatient and intensive outpatient care is 90 days at each care level.
Our team helps clients educate themselves about their condition and support them in meeting their goals. We work with clients to treat the root of substance use and stabilize their overall health, so they can achieve long-term recovery.

You know yourself better than anyone, so you stay in the driver’s seat during your care. Your schedule is flexible because your recovery plan is customized to what works best for you.
At Momentum, we treat you with compassion and respect your consent and freedom of choice. All we need is your willingness to talk and explore your motivation to change.
During your sessions, your counselor may use motivational interviewing. This special technique helps you become inspired and empowered to make your own positive changes.
You’ll also find a strong support network, which is key to recovery. Our staff reflects our local community, and some of them have personal experience with substance use. They’re a living testament to what clients can achieve. You can also connect with other clients for mutual sponsorship on your journey.
